Registering as a Patient
The doctors welcome new patients who live within our practice area.
Please bring your NHS medical card and proof of identity such as a utility bill and a current passport. New patients are automatically assigned to a named GP, although their first appointment will depend on availability and the reason for the consultation, eg whether urgent or routine.
All patients must complete a new patient registration questionnaire. Some new patients aged 15 and over may be also required to have a registration medical with a practice nurse or healthcare assistant although this will not delay their registration with the practice.
